• Patient Personal Information: As we provide services to our patients, we gather important personal details such as their Name, Address, and Date of Birth, along with other information that is classified as Protected Health Information (PHI). This information also includes data that describes the diagnosis and treatment of one or more health conditions associated with the services we are providing.
• Aspiring Employee and Current Employees: By enrolling to become a Citiscope Mobile Phlebotomist, you furnish us with your personal details such as your name, address, phone number, email, and certification expiration date. Additionally, we will collaborate with a credible third-party background verification service provider of your selection and share the information with them.

Payment Information:

• In order to process payments, patients provide their credit card details. However, Citiscope takes measures to safeguard your data by not retaining any payment information on its servers. Instead handled ny third party services such as Stripe, Square and Paypal.
• For payment processing and tax purposes, Mobile Phlebotomists provide their banking details and tax information.

• Have direct communication with both patients and mobile Phlebotomists such as calling to confirm booking.
• Deliver assistance to customers and handle any problems related to the services.
